To ensure timely processing, all referrals must include:

•        Date of referral
•        Referring GP’s name, address, provider number, and HealthLink ID
•        Patient’s full name, address, date of birth and contact number
•        Reason for referral

If available, please also include:

•        Patient’s Medicare number, private health fund details, and email address

Referrals must be reviewed and triaged by the doctors at Hollywood Gynaecology, before an appointment can be booked. Once triaged, our staff will notify the patient when to contact the clinic to schedule an appointment.
